Terms of Use — Patient Portal

These terms cover your use of the Port City Medical Patient Portal. By using it you agree to them. Port City Medical, LLC (“Port City Medical”, “we”) runs the portal.

1. Who may use it

The portal is for our patients and the people they choose to help them (a family member, caregiver or legal representative). Your sign-in link and code are for you — please don’t forward them. You are responsible for keeping your phone and e-mail secure. If you think someone else got into your account, call us right away and we will close the old sign-ins.

Use the portal only for yourself, or for a patient you are allowed to act for (as their caregiver, parent, guardian or legal representative). A patient who is a minor (under 19 in Alabama) uses the portal with a parent or guardian, who agrees to these terms for them.

2. Not for emergencies, not medical advice

The portal is not monitored around the clock. For a medical emergency, call 911. Nothing on the portal is medical advice; questions about your health or how to use your supplies belong with your doctor or with us by phone at (251) 443-7667.

3. Your information

Your health information is protected by law and handled as our Notice of Privacy Practices describes. We show you only your own records. Texts are sent only with your consent; message and data rates may apply; reply STOP to stop and HELP for help. Please don’t send card numbers or passwords in a message.

4. Keep us up to date

Please tell us when your address, phone, e-mail, insurance, doctor or needs change — we use your phone and e-mail to check it is you and to reach you, so an old one could send your notices to someone else. What you tell us on the portal is used to supply and bill your items, so it must be true and complete to the best of your knowledge.

5. Acceptable use

Use the portal only for your own care. Don’t try to see anyone else’s information, interfere with the portal, or use automated tools to access it. We may suspend access that is misused, and if we do, please don’t open a new account without talking to us.

6. Our content

The portal, its design, text, graphics and software belong to Port City Medical, LLC and are protected by copyright and other laws. “Port City Medical” and our logo are our marks. You may view and print pages for your own personal, non-commercial use; anything else needs our written permission.

7. Availability and limits

We work to keep the portal accurate and available but may change or pause it at any time, and it is provided “as is”. To the extent the law allows, Port City Medical is not liable for indirect or consequential losses from using or being unable to use the portal. Nothing here limits any right you have under law or under your insurance.

8. Your rights as a patient

Using the portal is optional — you can always reach us by phone, mail or in person. Your rights are explained in Your rights and responsibilities, Complaints and concerns and our Notice of Nondiscrimination.

9. Notices by e-mail and text

By agreeing, you accept that we can give you notices about the portal and your account electronically — on the portal, by e-mail or, if you said yes to texts, by text. You can always ask for them on paper instead, at no charge.

10. Changes and law

We may update these terms; the date below shows the current version. When they change, the portal asks you to agree to the new version the next time you sign in. Alabama law governs them.
