Our commitment to your privacy

It is our duty to maintain the privacy and confidentiality of your protected health information (PHI). We create records about the treatment and services we provide to you, and we are required by law to keep them private and to follow this notice as currently in effect. We may change this notice; the current version is always available here and, on request, as a paper copy from our Privacy Officer, Nazmi Ozokur.

How we may use and share your information

  • Treatment: providing the supplies and services your physician ordered, and coordinating with your doctors, hospitals, home health or hospice.
  • Payment: billing, claims, prior approval, and determining eligibility and coverage with Medicare, Medicaid or your insurer.
  • Health care operations: quality checks, responding to complaints, compliance and audits, staff training and evaluation.
  • Reminders about your supplies and resupply; information about treatment alternatives and health-related services.
  • With a family member, friend or caregiver involved in your care or picking up supplies — you may object.
  • As required by law: public health, health oversight, law enforcement, lawsuits, coroners, workers’ compensation, national security, and to prevent a serious threat to health or safety.

Any other use — including marketing, the sale of your information, or psychotherapy notes — requires your written authorization, which you may revoke in writing at any time.

Your rights

  • Inspect and copy the PHI in our records (we may charge a reasonable fee for copies). Ask in writing.
  • Request an amendment when you believe information is wrong or incomplete; we may deny the request in some cases and will tell you why.
  • An accounting of disclosures we made in the six years before your request, other than for treatment, payment, operations or with your authorization. The first accounting in a 12-month period is free.
  • Request restrictions on how we use or share your information. We must agree when you paid in full out of pocket for an item and ask us not to tell your health plan.
  • Confidential communications by other means or at another address.
